Northern Lincolnshire and Goole Hospitals NHS Foundation Trust

Northern Lincolnshire and Goole Hospitals NHS Foundation Trust was established as a combined hospital and community Trust on 1st April 2001 and achieved Foundation Status on 1st May 2007. It was formed by the merger of North East Lincolnshire NHS Trust and Scunthorpe and Goole Hospitals NHS Trust and operates all NHS hospitals in Scunthorpe, Grimsby and Goole. Its name reflects the wider geographical area in which the Trust is a major provider of health care.

1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

  • Northern Lincolnshire and Goole Hospitals NHS Foundation Trust is committed to improving the care of people with dementia and the support to their carers
  • Northern Lincolnshire and Goole Hospitals NHS Foundation Trust works closely with local partners in the delivery of dementia action plans
  • Implemented Dementia Champions
  • Implementation of ‘My Life’ (person centred planning tool), in conjunction with the local Dementia Forum
  • The Trust has dedicated Mental Health Liaison teams for Older People for improving the care of patients with dementia, delirium and depression
  • Introduction of Dementia Implementation Group to ensure the Trust’s Dementia Delivery Plan is achieved
  • Dementia, delirium and depression pathways have been developed
  • The Dementia CQUIN has been implemented to improve early diagnosis

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

  • The Trust is spilt over 3 sites, which includes working with 3 different Mental Health teams and 3 different commissioning groups
  • Releasing staff for education and training
  • Resourcing of the training programmes
  • To ensure the work around dementia is given a high priority and that management and clinical staff are aware of the needs of this group and that the services delivered need to be tailored
  • Demographic change, ensuring we are prepared for the high activity and demand
  • Ensuring CQUIN is delivered
  • Embedding behaviour and culture change across all levels of the organisation
  • Financial constraints to enable the delivery of key objectives.
  • Delivering the objectives relating to dementia whilst continuing to deliver other organisational performance indicators.
  • Being able to implement dementia friendly principles within the existing infra-structure.
  •  Our hospitals will require refurbishment to ensure we are offering a dementia friendly environment
