1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We want to be part of the local group helping promote an understanding locally of those living with dementia and their carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We are extremely fortunate as the Group our Home belongs to are happy we give time and resources to this activity.

2. Actions

  • Become part of the Dementia Action Alliance for area

    Contact the Co-ordinator, meet with them and complete paperwork. Attend the meetings organised by our local DAA and join the steering group if possible

  • Run a Dementia Memory Cafe

    Using our facilities within the Home and discuss with Admiral Nurse based in St Peters Hospital whether wish to join forces

  • Provide a regular Carer Support/Pamper Event

    Using our facilities within the Home, identifying and enlisting the support of local businesses for donations, providing refreshments for the day and promoting locally
