1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are taking on more and more patients with a diagnosis of dementia so we would like to join the DAA to be part of a network that can provide us with information to share with them and their families.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We have an in house Reviewing Officer who will assess effectiveness of our action plan and feedback to the team.

Changes will be actioned by the reviewing officer and changes made to care plans where applicable.

2. Actions

  • Dementia Awareness

    We will encourage existing staff and new staff to attend a Dementia Friends session.  We will also look at the possibility of a member of staff becoming a Dementia Friends Champion to allow us to hold sessions internally.

  • Work in partnership with local organisations

    We will develop relationships with other organisations, charities and service providers to signpost clients where we are not able to provide a specific service for them.  We will promote opportunities to clients and family members where appropriate.

    We will work closely with the Dementia Adviser service in Bracknell Forest to ensure that we are aware of Bracknell specific services and who to turn to for further help.

  • DAA Membership

    We will promote our DAA membership, encouraging partner organisations to also sign up.  We will support the local DAA in our area and commit to attending meetings where possible and assisting with the LDAA group action plan.
