1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

It is vital as the NHS provider we are able to reach as many local people as possible to inform them of services available, what support is around and who they can go to for information.

 Need to avoid hospital admissions if not required.

Provide support and make reasonable adjustments in attending out patients 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Time constraints to attend meetings but would keep in regular contact with the group through email
