1. Action Plan

Make a difference to the lives of people living with Dementia by increasing knowledge and understanding, to help people live well with Dementia.  We will support the work of the DAA and see Great Yarmouth become a Dementia Friendly Town.

    What we are going to do: Look at opportunities to raise funds to enable media cover and other suitable marketing. Use fetes, work activities to promote and fund raise.

    What we are going to do:

    Use media, businesses, local community to enhance knowledge and understanding, support and help individuals to receive appropriate aid. Commit time to meet and discuss appropriate actions to take the group forward in Great Yarmouth.

    What we are going to do:  Promote Dementia awareness through local businesses and meetings and working with the community. The Care homes has a lot of stakeholders who we could use to do this with.

    What we are going to do      we are going to carry on raising awareness by holding awareness days in Gt Yarmouth , the J U P H , The Forum in Norwich and other locations including Norwich Station where we will put on a display of memorabilia  which attracts a lot oif attention and therefore ‘raises awareness’

    We are at the moment working with Coastal Village Practices and East Coast Practice  holding informal meets and get togethers for people living with dementia or newly diagnosed patients . at these we offer practical  advice and also have a wide range of information available for people to take away . this will continue

    Provide music based reminiscence activities on the second and fourth Thursday of each month in a safe, supportive environment.

  • Norfolk Constabulary, Great Yarmouth (1) Herbert Protocol

    The police in Great Yarmouth will set up a system to register a Herbert Protocol in cases when someone with dementia, who lives in their own home, and is at risk from wandering or has been missing and later found.    A copy will then be kept in the home and it will be tagged to the Police Control Room.

    To consider how the public enquiry office at Great Yarmouth Police Station can be made dementia friendly when it is renovated.

    We will identify members of frontline Policing to become Dementia Champions; they will receive training in Dementia awareness and will be a source of information and support to colleagues, and can facilitate training for teams.

    Update the CCG’s section of the website on dementia to ensure there are the links and contact details to other organisations which people with dementia and/or their families/carers can access

    Undertake a dementia assessment of buildings Signage etc. Working towards making our space more Dementia friendly to our customers.

    Continue to provide space for partners working with people with dementia to engage and support their clients. Currently host a monthly Dementia café and carers support group.

    As part of Norfolk County Councils commitment to the Dementia Action alliance there will be a focus on the promotion of the dementia champion role within the Eastern locality area.

    As part of Norfolk County Councils promoting independence strategy workers are required to support individuals to use their communities and close networks to improve and manage their circumstances. As part of the dementia alliance Norfolk County Council will endeavour to continue to support individuals experiencing dementia and those that care for them.

  • Norfolk County Council Adult Social Services (3) Work in a multi-disciplinary way to promote a dementia friendly community

    Norfolk County Council works closely with its partner agencies including the statutory and voluntary sectors. As part of the dementia action alliance Norfolk County Council will continue to work with these agencies and promote the dementia alliance to support the development of the initiative.

    We are going to carry on doing as much reminiscence training as we can which makes carers and cared for’s lives that little bit easier we have already trained between 5 and 600 candidates from all walks of life and all, parts of Norfolk and Nth Suffolk including the N H S Dementia team in the J U P H Gt Yarmouth .
