1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a GP surgery, our aim is to provide early and accurate diagnosis, and to facilitate appropriate and compassionate care and treatment. We actively support each individual's right to be a partner in their care and for them to be provided with the choice of taking part in dementia related research.

We would like to improve the care our patients recieve by working more closely with local partnership organisations.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Challenges include funding, lack of resource and lack of understanding. We need to improve partnership working to ensure our response to dementia is consistant throughout our community.

  • Flag patients with Dementia

    Put a flag on the clinical system when a patient is known to have dementia, so that front facing staff can alter their approach if necessary. Action by 30.12.17
