1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are very proactive when it comes to our dementia patients. We have recently moved to new premises and we had assistance from the Alzheimers Society to ensure that the surgery is as dementis friendly as possible. We have a lead dementia nurse and our Patient Participation Group and staff have had Dementia Friends training sessions. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

I think its that the very nature of the disease means that patients can progress very quickly.
