1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To support the quality of life of people living with Dementia and their carers. 

To identify a practice Dementia Champion.

Supporting early diagnosis and providing support.

Increase awareness among staff and patients.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Setting aside time for staff training.

Encouraging patients ad carers to engage with the service.

Managing expectations of patients.

Lack of clear structure for referring people with dementia and their families to support services.

2. Actions

  • Dementia Awareness

    Staff, practitioners and members of the PPG to become Dementia Friends and wear their Dementia Friends Badges.

    Identify a members of staff to attend training to become a Dementia Champion and be responsible for dementia within the practice.

    Sponsor events in the local community to raise awareness of dementia, such as coffee mornings with the attendance of a member of the Alzheimer's Society.

    Display up to date information and literature in an easy and accessible manner.

    Provide information packs to newly diagnosed patients and/or family members.

  • Appointments

    Ensure staff are able to identify and support patients with Dementia by flagging patients on the clinical system.

    Use appointment reminder tool to send text messages to patients or their nominated carer.

    Be flexible around appointment times, avoiding early appts, etc.
