1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

WE ARE A LOCAL GP PRACTICE WHO PROVIDE A SERVICE TO PATIENTS  IN 6 LOCAL RESIDENTIAL AND NURSING HOMES AND A RELATIVELY HIGH ELDERLY POPULATION. WE AIM TO OFFER INDIVIDUALIZED CARE WITH ACCESS TO APPROPRIATE PROFESSIONALS AND RESOURCES TO ALL PATIENTS WITH DEMENTIA AND THEIR CARERS.                             

WE HAVE NAMED PROFESSIONALS WITHIN THE PRACTICE WHO ACT AS A POINT OF CONTACT FOR PATIENTS WITH DEMENTIA (NAMED GP, PRACTICE MATRON  ADMINISTRATOR)                                                    

ALL CLINICAL AND ADMIN STAFF HAVE RECEIVED TRAINING AND ARE 'DEMENTIA FRIENDS'.                                                                                    THE CLINICAL ENVIRONMENT HAS BEEN ASSESSED AND A QUIET AREA CAN BE ACCESSED FOR DEMENTIA PATIENTS WHO REQUIRE IT AWAY FROM THE BUSY RECEPTION AREA.                                                           SIGNAGE HAS BEEN CHANGED IN THE RECEPTION AREA TO ENSURE IT IS EASIER TO UNDERSTAND.                                                                          UP TO DATE LITERATURE IS AVAILABLE FOR DEMENTIA PATIENTS AND THEIR CARERS.                                                                                SURGERY STAFF ARE ABLE TO PROVIDE LINKS WITH SOCIAL CARE + VAST (VOLUNTARY SECTOR) FOR PATIENTS WITH DEMENTIA AND THEIR CARERS.                                                                                               THE SURGERY HAS A REGISTER WHICH RECORDS ALL PATIENTS WITH DEMENTIA AND ANY RELEVANT ADDITIONAL INFORMATION SUCH AS SPECIFIC NEEDS WHEN ACCESSING HEALTHCARE AVAILABLE FOR ALL SURGERY STAFF TO ACCESS TO ENSURE PATIENTS NEEDS ARE MET.  

 ALL DEMENTIA PATIENTS HAVE A DISCREET ALERT ON THEIR MEDICAL RECORDS IN ORDER THAT STAFF ARE AWARE TO OFFER A LONGER APPOINTMENT/HOME VISIT                                                                      

ALL PATIENTS WITH DEMENTIA ARE OFFERED AN ANNUAL REVIEW                                                                                                               

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

CHALLENGES INCLUDE HAVING THE TIME FOR ONGOING TRAINING AROUND THE ISSUES OF DEMENTIA.

PRESSURES AROUND APPOINTMENT AVAILABILITY

CHANGES IN STAFF AND ENSURING ALL STAFF RECEIVED THE NECESSARY UP TO DATE TRAINING INCLUDING 'DEMENTIA FRIEND'.

    Ongoing review of environment, ensure availability of quiet area as required.

    Ensure literature readily available, check stock and ensure latest literature available.

    Update practice website to ensure information available.
