Grange Park Surgery

We are a GP surgery of 6’700 patients. We have 6 part-time G.P’s and 2 full time G.P’s. We currently have one GP that will visit our care home, Burley Hall on a weekly basis to review our patients. We have under half of our Dementia patients living in sheltered accommodation and care homes. Which means we have managed to continue to care and help keep over half of our patients in their own homes living independently.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

To continue to support our patients, families & carers as well as continuing to work closely with our care homes to help them live well with Dementia.  To continue to  work closely with our PPG group and to continue to attend meetings with in the community with Burley-In-Wharfedale Dementia Action Group. We will continue to educate and identify where training is required to continue being a Dementia Friendly with in our community. More collaborative working within the community.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Staff shortages – sickness and absence could prevent being able to attend meetings.

  • Improve our knowledge and understanding of Dementia

    Dementia friendly training has been delivered recently to most employees.  Further ongoing training and updates to continue.  We are also liaising with the Burley Dementia Group in how we can bring awareness of Dementia to the community.

    Our core team have already taken part in DF awareness sessions. Our Manager and Volunteer Coordinator will arrange for dementia awareness to be built into the event staff and volunteer training.
