1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

The Victoriana has 34 fully furnished rooms for residents, some of which have en-suite facilities. Both of our lounges overlook the peaceful garden and its beautiful scenery. At Victoriana Care home, all our residents have a person-centred care plan and a person-centred approach to care.

We engage with residents’ families to ensure we have a thorough understanding of their life history to help them engage in meaningful activities.

We support access to the local community through local outings such as to the library.

We aim to create a “homely” home which is Dementia Friendly and where our residents can feel safe and secure.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We can find difficulties finding resources to meet our aspirations but we continue to work on this.

We would like to work on developing our skills to motivate engagement in activities.
