1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
The Victoriana has 34 fully furnished rooms for residents, some of which have en-suite facilities. Both of our lounges overlook the peaceful garden and its beautiful scenery. At Victoriana Care home, all our residents have a person-centred care plan and a person-centred approach to care.
We engage with residents’ families to ensure we have a thorough understanding of their life history to help them engage in meaningful activities.
We support access to the local community through local outings such as to the library.
We aim to create a “homely” home which is Dementia Friendly and where our residents can feel safe and secure.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
We can find difficulties finding resources to meet our aspirations but we continue to work on this.
We would like to work on developing our skills to motivate engagement in activities.

Dementia Awareness and Training
We will hold a Dementia Friends Information Session in September 2017 and publicise this to staff, resident’s family and friends.
We will review the free online dementia training resources and use these to supplement face to face training and review in individual staff training plans.

Supporting access to a timely diagnosis
We ensure Mental Capacity Assessments when developing resident’s individual care and treatment support plans. We also identify residents with memory or cognitive impairment for referral to their GP for diagnosis.
We will review the DeAR-GP method for assessing residents and share with GPs current guidance on diagnosis pathways for patients in full time residential care.

Making Community connections
