1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Improve pateints health and well being and care/support received after diagnosis.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Enabling the correct care is provided and patients/carers are aware what help is available.

  • Improve care for patients after initial diagnosis

    Provide patients/carers details of aftercare support available and how to access the information.
