1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

At St Anne’s we support each resident to have an individual care plan to meet their specific needs. We work alongside resident’s families to understand their like profile so we can engage with them more effectively.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Our difficulties will relate to time management around training but we will develop resource planning tools to enable us to meet these objectives.
