Tamworth House Medical Practice
Tamworth House Medical Centre is a partnership of five doctors, four working full-time and Dr. Santina LaPorta working part-time on Mondays, Tuesdays and Wednesdays. As a practice we try, not only to treat illness, but also to promote general good health and well being in our patients. We endeavour to treat all our patients as individuals, with needs that are not only physical but also emotional, irrespective of race, creed, colour, gender, sexual orientation or religion. The practice is part of Merton Clinical Commissioning Group operating under the Personal Medical Services contract.

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
We aim to provide the higest standards of healthcare and make sure our services are accessible for people living with dementia and carers in the local area.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Dementia Friend sessions for practice staff
Organise for a Dementia Champion to deliver a Dementia Friends session to practice staff to help them better understand dementia
Keep stock of information about dementia
Ensure that information about dementia is avaliable for patients in the waiting room
Host an Alzheimer's Society Information Stand
Organise for the Alzheimer's Society in Merton to host an information stand to raise awarness of dementia and local support services in Merton
