1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Bluebird Care offer bespoke care packages to all our customers to meet their specific needs, we constantly liase with others to ensure the smooth delivery of care. We provide ongoing training and continued support to all care assistants.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We have a limited amount of time to contend with and different peoples understanding of Dementia or even the lack of understanding by customers, families and care assistans.

2. Actions

  • Dementia Friends

    Bluebird Care promote the Dementia Friends Campaign. It is included as part of all new Care Assistants Skills Assessment training.

  • Supportive

    Bluebird Care offer continued support to all Care Assistants, Customers and their families. People are able to come into the office or telephone with any issues they may have or we are able to go into the customers home to support and advise accordingly.

  • Raising Awareness

    Bluebird Care will continue to raise awareness of Dementia, by passing on knowledge to care assistants, customers and their families about the difficulties a person living with Dementia may have or the difficulties a family member/carer may have supporting the person living with Dementia.

  • Person Centred

    Bluebird Care plan along side our customer. Care plans are person centred, they are compiled from information given to us directly from the customer which ensures we are meeting their individual needs in precisely the way that they would like, therefore enabling them to have control of their own lives.
