1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We aim to provide the higest standards of healthcare and make sure our services are accessible for people living with dementia and carers in the local area.

2. Actions

  • Raise awareness of dementia amounst patients

    Firstly, we will host an Alzheimer's Society information stand at busy times to provide information about dementia and avaliable support services to those interested in learning more, those with concerns about their memory and those people living with dementia and their carers.

    Secondly, we will make sure that we have information in waiting rooms for people to look through.

  • Host a Dementia Friends information session for staff

    Organise for a Dementia Champion to come in and deliver a Dementia Friends information session to staff.

  • Provide patients with the Alzheimer's Society Dementia Guide

    We will order 25 free copies of the Alzheimer's Society Dementia Guide and give them to GPs who can then hand them out to those patients in need of informaiton.
