1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Tesco Martins Heron would like to become a member of the Bracknell Forest DAA in order to help their customers and provide an easier shopping experience for both people living with dementia in the community and their carers.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Tesco Martins Heron will review their action points in two ways, on their social board and also at their Monday managers meetings.
