Astbury Manor - Avery Healthcare

Avery Healthcare operates residential, dementia and nursing care homes throughout the UK, with a focus on the highest possible standards of living for our residents and peace of mind for family and friends alike. Astbury Manor is located in Bracknell to promote the rights of people with Dementia. Astbury Manor is the first care home in Bracknell Forest to become part of the alliance.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Interaction with the local community, ensure that the training and presence of Astbury Manor is visible, and promoting the wishes, aspirations and needs of people on their dementia journey. Astbury Manor will have a dedicated Memory Lane community within the home, designed and laid out specifically to meet the needs of people on their dementia journey.

Integrate within the Bracknell Community, and develop links for residents to access as and when they desire.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Feedback from colleagues, other organisations within the Dementia Action Alliance, and feedback from families, friends, staff working alongside people on their Dementia Journey. Regular contact with peers within the DAA, so that all services work in harmony for the development of services. Identify current issues relating to deficient in current services for people on their dementia journey.

2. Actions

  • Engage with local services and promote the rights and perspectives of those living with Dementia.

    Work closely with other agencies and services to ensure people with Dementia are heard, and are able to influences services for the future.

  • Support existing services in Bracknell/Berkshire for the future, to maximise their success. Continue to educate the staff at Astbury Manor to develop skills, and knowledge of Dementia, and ways to assist people to maximise their quality of life.

    Use local services as much as possible. Work with other agencies and contribute to the future direction of Dementia services within Bracknell for the future.

  • Promote the development of Dementia friends within the care home, teaching staff, and residents families about this training.

    Hold Dementia friends sessions within the home and open home to the community for these sessions. First session scheduled for the 26th June 2017

    Dementia friends champions to be identified within the Astbury Manor team members to attend the Dementia Friends Champion Induction Days sessions, so that they are able to deliver this training within the home in the future.

    Astbury Manor is able to offer meeting space for future DAA meetings as and when required under prior arrangements.
