The practice are a Dementia Friendly practice working towards gold standard in the Alzheimer's toolkit. Raised £350 last week with a raffle and bake sale for the Alzheimer's awareness week.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Working towards gold standards and involving the PPG. All staff and PPG members have become dementia friends following training.

The practice are currently working on information packs for patients and family/carers to inform them of ways the practice are helping with the care.

Fund raising 2yr in a row for the alzheimers week has just taken place

Practice steps have been taken already to help identify patients and their diagnosis so that appropriate help/service can be applied when contact is made with the practice

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The vast continous changes to the NHS mean that time is currently a restraint but we are making small steps to implement changes with the way that the records are kept and updated for dementia patients and making information available to carers.

2. Actions

  • Not attending appointments

    Monthly searches for patients with dementia who have DNA'd appointments either at the hospital or surgery will be carried out and reviewed regularly.

    Reception staff to telephone patients to remind them of their appointment at the surgery.

    The practice have just completed their 2nd year of supporting the Alzheimers awareness week but selling cakes and holding a raffle.

    £350.00 was raised this year.

    The practice are offering dementia patients the opportunity to inform the practice of who should be contacted in the event of an emergency
