1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

I see our role as helping to provide a healthcare environment which is easy to navigate for those with dementia. Further, to be able to signpost effectively to other services which may be helpful to dementia patients and their families.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Time constraints are our biggest hurdle - PrimaryCare is under a lot of pressure at the moment. There are a lot of mandatory training requirements imposed on us, and so finding time to educate staff about the DAA will be challenging given this.

2. Actions

  • Train staff to be "Dementia Friends"

    Give all staff option to do dementia friends training.

  • Put up Dementia friendly signage around the practice

    Put up Dementia friendly signage around the practice

  • Replace clocks with ones with AM/PM

    Replace clocks to ones which show whether it is morning or afternoon.

  • Let patient's know we have joined the scheme

    Use website and local magazine to promote our involvement.
