1. Action Plan

Working with the elderly and supporting residents with dementia, I recognise that there is a lot of support available locally. We will keep up to date with these services and keep our residents involved to promote independent living.

2. Actions

  • Raising Awareness of Dementia amongst residents

    As I am a dementia friend champion I will organise sessions at our local schemes.

  • Facilitating the early intervention and detection of dementia

    We will ensure that residents and family members are signposted to the correct specialists and support them through the process.

  • Working in partnership with local organisations

    A member of staff will regularly attend DAA meetings and keep up to date with local information in the area.
