1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Promoting healthier lifestyles amongst older people to reduce the risk of developing dementia, and promoting these opportunities to those with dementia

Help ensure people with dementia or those closest to them are better able to keep in touch with family/friends or take part in activities in the community

Promote awareness of needs of people with dementia in organisation

Make changes to the services, facilities or products to make it easier for people with dementia or their families to access or use them

Promote awareness of needs of people with dementia to others, including public

Work with people with dementia and families to feel involved in monitoring & reporting outcomes

All projects run by the foundation are monitored and reported on against agreed outcomes.

Information & help in understanding more about dementia or memory loss

Advice about encouraging staff & others to be “dementia champions”

Help providing training for staff in dementia-awareness

Help providing training for staff in working or engaging with people with dementia
