1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Key role in delivering care to those living with and those caring for those with dementia. Contact through GP, Receptionists, Nurses etc.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Keeping everyone up to date, getting new members of staff trained as dementia friends. Keeping the initiative moving.
In my role as Community and Care Co-ordinator I am in touch with Alzheimers Society regularly for help. I am a volunteer also for singing for the brain (personally) I can access help and advice.
