1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Through our existing links with other third sector organisations, healthcare professionals and statutory services. As well as our work with clients/local residents who might be living with dementia or caring for those with dementia.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Most of our interactions with the public/clients take place within their homes, which could present some additional challenges when supporting those with dementia. We will overcome this through our existing home visiting policies and procedures, and by seeking external support if necessary.

Staff will attend a Dementia Friends session so we can all be more aware of dementia and how it can affect people in our interactions.
