1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We want to help promote awareness of dementia within the city.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

I would refer any one who needed more assistance to Alzheimer's Society.

  • Raise awareness of dementia

    I will wear my Dementia Friend badge and hand out leaflets about sessions.

  • Dementia Support Group

    I will set up a a Dementia Support Group in Blackfield.

  • Share the work of Dementia Friendly Southampton

    I will add the logo to my email signature and share posts on social media.
