1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

It’s part of the Church’s mission to be welcoming and inclusive. By reviewing the needs of all groups who use the Church and in conversation with Dementia Friendly Leominster we will establish how best to serve the community.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Getting training and information into place.

    Do an audit on the building to see how dementia friendly it is.
