1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Government initiatives influence commissioner decision making.  Ask people what is helpful when operating services

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Shared priories with other health and social care providers.  We will get over these by using the powerful voice of people with dementia and their carers.

2. Actions

  • Create Dementia Friends

    We will make Dementia Friend Information Sessions available for all staff.

  • Establish a user review group

    Establish a group of people with dementia and carers to be involved with developing quality services

  • Develop our Dementia Pathway

    We will use our local third sector support services to further develop a supportive dementia pathway
