1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
I know services are designed around me and my needs - Ensuring that people with dementia are understood; our service will support them with dental hygiene. We will treat them with dignity and respect and ensure they are supported when accessing services.
I have knowledge and know how to get what I need - We will ensure that all information is communicated effectively and ensure it can be digested and re-digested.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
