Bracknell Forest Dementia Action Alliance

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We have many patients with a diagnosis of dementia and we would like them to feel safe when attending the surgery and assured that we now have a greater awareness of their needs by joining the Dementia Action Alliance

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review the action plan put in place at our PPG meetings and through the use of the patient survey

2. Actions

  • Dementia Friends

     Encourage all staff to become Dementia Friends by holding a one hour awareness session for them to attend.

    Where appropriate seek further training for staff.

    Provision of up to date literature and information displayed in waiting areas, and awareness of local support services to signpost individuals to.

  • Dementia Friendly Environments

     Using the DAAs environmental checklist we will look to make adjustments to our Practice to ensure it is as dementia friendly as possible, considering factors such as signage, seating, display boards, flooring and automated arrival screens.
