Forest Health Group

Forest Health Group is a long established GP surgery located across two sites in Bracknell. Both sites are located conveniently within easy reach for patients and have car parking available. The practice offer a large variety of services including minor surgical services and contraceptive services. The practice aim to provide a caring and patient-centred service to our patients. The partners are Dr. S.K. Ball, Dr. M.T. Kittel, Dr. S.D. Lewandowski & Dr. R.A. Boyce.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

As a caring GP practice fully aware of the needs of patients with dementia and their carers we would like to be involved in local initiatives to help support them further.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will add the review of our action plan to our team meeting agenda which is covered at BASE events.

We will also review at out PPG meetings.

2. Actions

  • Dementia Friends

    We will encourage all staff to become Dementia Friends by holding a one hour awareness raising session for them to attend. 

    Where appropriate we will seek further training for staff.

  • Dementia Awareness Week

    We will help to raise awareness of dementia during Dementia Awareness Week by converting our primary information table into solely dementia related materials.

  • Provision of up to date literature

    We will provide and display up to date literature from the DAA and the Alzheimer's Society in our waiting rooms and around our practice.

    Arrange for people with dementia and their carers to come and do an environmental review of the surgery to ensure that we are as dementia friendly as possible.
