1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We aim to identify patients affected by dementia and their carers and be alert to their needs.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We are a busy GP surgery and may face time restraints but we will endeavour to offer appropriate guidance whenever possible

2. Actions

  • Dementia Friends

    We will ensure that many of our employees attend a Dementia Friends session either face to face or online. This will help to increase their knowledge and understanding of dementia so that they feel better equipped to deal with any customers displaying the signs and symptoms of the disease

  • Dementia friendly signs

    We will use some dementia friendly signage within the surgery to indicate for example, The Nurses Treatment Room and the toilet facilities.

  • Information for people with dementia and their carers.

    We will display information in the surgery to signpost people with dementia and their carers to both national and local services which offer support and advice.
