Holly Medical Group

We are a GP surgery in Jesmond dealing with a wide range of health problems. We also provide health education, offer advice on smoking and diet, run clinics, give vaccinations and carry out simple surgical operations. GPs usually work in practices as part of a team that includes nurses, healthcare assistants, practice managers, receptionists and other staff. Practices also work closely with other healthcare professionals, such as health visitors, midwives mental health services and social care services.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will aim to provide as much information as possible by way of posters, newsletter, etc.  Staff awareness, training.  We can also provide a quiet room for those patients who need it.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

It is difficult to be able to spare staff for training but we will endeavor to do this online.

2. Actions

  • Dementia Friends

    We will ensure that employees who have not received more detailed training  attend a Dementia Friends session either face-to-face or online.  This will help to increase their knowledge and understanding of dementia so that they feel better equipped to deal with any patients displaying the signs and symptoms of the disease.

  • Display Information

    We will display the JDAA Newsletter and dedicate a display space for information about activities and  support sessions for people with dementia and their carers available in the area, including the Jesmond Library Information Hub

  • Dementia Awareness Raising for Staff

    We will have a copy the Alzheimer’s Association  Customer Facing Staff Guide available and bring this to the attention of our staff as well as looking into more specifically GP focused guides.
