Bracknell Forest Dementia Action Alliance

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We recognise the importance of dementia and its increasing prevalence.  We should be part of an alliance to improve both care and diagnosis.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review the action plan put in place as part of the monthly practice meetings.

2. Actions

  • Dementia Awareness

    We will encourage all staff to become Dementia Friends by holding a one hour awareness session for them to attend.

    Where appropriate we will seek further training for staff.

  • Dementia Friendly Environmental Review

    Using the DAA's environmental checklist we will look to make adjustments to our practice to ensure it is as dementia friendly as possible.  We will consider factors such as signage, display boards, flooring and automated arrival screens.

  • Improve Diagnosis Rates

    Using the 'audit tool' we will check we have coded patients correctly and included them on the dementia register.  We will ensure that we follow the pathway on the map of medicine to ensure that where appropriate people are diagnosed in a timely way.
