Binfield Surgery

The Binfield Practice is a well-established, respected surgery, situated in Binfield on the outskirts of Bracknell. We are part of the Bracknell & Ascot Clinical Commissioning Group. The partnership is a GMS practice and services over 10,500 patients. The partners are Dr Robert Koefman, Dr Jeremy Platt and Dr Vitaliy Herhel.

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Binfield Surgery have joined the Dementia Action Alliance so that staff have a better understanding of Dementia and are aware how to support patients and their carers who have a diagnosis of dementia.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

The action plan will be reviewed in the monthly team meetings to ensure that actions are up to date and relevant.

  • Dementia Friends

    We will hold a Dementia Friends session to raise awareness of a dementia for all staff to attend during the BASE day on 9th May at 1:30pm

  • In House Dementia Champion

    Nominate one member of staff to become the Binfield Surgery Dementia Champion.  This champion will keep the rest of the surgery abreast of the most up to date information relating to dementia, attend the DAA meetings on behalf of the surgery and feed back and also attend the Dementia Friends Champion Induction in order to be able to deliver the Dementia Friends Information Sessions to any new staff.

  • Dementia Friendly Environmental Review

    In association with people with dementia and their carers we will organise a walkabout at the surgery to review the internal environment and implement any suggestions made. 

    We will ask our own patients who have a diagnosis of dementia if they would like to be involved in the walkabout and suggest any changes to be made.
