1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Personnel & Care Bank have dedicated and experienced Care managers who will complete an initial assessment including a person centred and detailed care plan, taking into account the persons cultural or religious preferences. The care manager will then visit to ensure the expectations of the care package and the needs of the person are continually being met.

Also ensuring that we are working together with other professionals and family carers. We engage in risk and work with the person and family carers to manage the risk.

We help people with dementia to access services, appointments and social activities. We would like to work closely with the Alzheimer’s society and signpost and raise awareness to people with dementia and there family

Personnel & Care Bank has support workers who show respect and dignity, understand person centred care and promote independence and choice. We also support specialist dementia training for all Support workers and are in the process of encouraging all support workers to become dignity champions and dementia friends.

We would like to have a portal of information at our office for care staff to information and knowledge in dementia care.

Personnel & Care Bank have also signed up for the dementia pledge.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Care packages that are set up with people with dementia where we have a time limit of 15 minutes or 30 minutes per call.

Ensuring that all Support workers share the company’s values and beliefs when delivering the service.

Ensuring attendance at training sessions as these are not mandatory and are unpaid sessions.

2. Actions

  • Dementia Friends support workers

    We are currently waiting to complete Dementia Friends Champion training, so that we can hold sessions in our office for all support workers.

  • Hold Dementia Friends sessions

    Hold Dementia Friends sessions for service users, family and carers and any of our service users who would also like to attend.

  • Care plans and life story books

    Ensure detailed and person centred care plans and life story books.

  • Limit the amount of support workers

    Limit the amount of support workers to ensure continuity and relationships with service user and family carer.

  • Encourage dignity champions

    Encourage all support workers to become dignity champions – they can come into the office and use the internet if required.

  • Specialist dementia training

    Support specialist dementia training for all support workers

  • Delivery of care with respect and dignity

    Ensure the delivery of care is done with respect and dignity and promote independence – all support workers are spot checked on a regular basis and have supervision sessions and appraisals where this is discussed. We visit our service users regularly.

  • Social activities

    To be able to support people with dementia more with social activities.

  • Dementia Pledge

    We have signed the dementia pledge and are committed to delivering the possible care.

  • Life Story Books

    All Service User's to have life story books.
