Pall Mall Surgery

There has been a doctors’ surgery in Pall mall, Leigh on Sea since the war, starting as one consulting room and a waiting room which has steadily grown. Since 2015 we have become a GMS (Generel medical services practice) and provide our services under contract to the southend CCG. The practice retains its name of The Pall Mall Surgery to benefit from the reputation we have developed over many years of local practice

1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We are transforming Pall Mall Surgery into a dementia friendly surgery using the iSPACE method and accreditation

All staff within the surgery will become dementia friends including GP’s, Nurses and other clinical staff, reception and administration staff.

We are developing and improving communication and joint working between health, social care and the community. 

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Dedicating time to the iSPACE project within our working day

Encouraging all staff to get involved with the project

 Communicating our working towards becoming a dementia friendly surgery with staff, patients and visitors

  • Encouraging all staff to get involved with iSPACE project

    All practice staff to become Dementia Friends. Through time to learn opportunities all practice staff will become dementia friends and monthly updates from the champions of progress

    2017 - Fourth Quarter Update

    We are all working together to be aware of any missed appointments within the Surgery by patients who have Dementia so that the GP can then follow up on the next step available for them 

    Dementia packs are handed out at Reception upon request of patients or if directed there by the GP 

    We have a Dementia board in the Surgery giving iinformation on day centres that can be visited and also information on carers 

  • Dedicating time to the iSPACE project

    Pall Mall has identified 2 dementia champions to lead the project. As both champions have full time work commitments within the surgery, time needs to be agreed with managers to dedicate to the project.

    iSPACE is a number of actions designed to improve the patient experience through delivery of a set of improvements in care planning, communications and awareness of dementia for staff in primary care settings.

     I – Identify a dementia champion,

     S – Staff Training,

    P – Partnership working with patients, carers, and the voluntary sector,

    A – Assessment and oversight of use of anti psychotics

    C – Care planning and use of tools such as ‘This is Me’,

    E – ensure the environment supports people with dementia and other disabilities

  • Communicating the progress of the working towards becoming a dementia friendly surgery with staff, patients and visitors

    The champions will create a dementia board which will explain the concept if iSPACE and identify the 2 champions. There will be a monthly written update to GP’s staff, patients and visitors, PPG  and updates on the surgery’s website.

    As an iSPACE dementia friendly practice we will work with the Commissioning Officer for Dementia and Southend Dementia Action Alliance lead to promote and support the creation of Dementia Friendly GP practices throughout the Borough
