1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Ringmead Medical Practice wish to become part of the Bracknell Forest DAA because as health professionals we are in a position to make a difference for local people living with dementia by supporting them in a trusted environment.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review our action plan at our monthly team meetings and also we hope that our efforts will be reflected in our patient surveys.

  • Environmental Audit

    We will invite people living with dementia and their carers into both practice sites and ask them to give feedback regarding our internal environment and suggestions of how this could be improved

    We will hold a Dementia Friends session for all staff in our 7th March and 9th April team meetings

  • In House Dementia Champion

    We will nominate one member of staff to become the in house dementia champion.  This member of staff will attend the one day Dementia Friends Champion induction so they are able to hold information sessions for new staff members or those that miss the March and April sessions.  This person will also attend the DAA meetings where appropriate.
