1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
seAp have joined the DAA so that they can network with other organisations in the community to best support people living with dementia.
seAp wish to ensure that they have complete best practice for people that they work with when supporting people with dementia
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
seAp will review their action plan in their team meetings.
