1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We run a memory cafe once per month and we are hoping to expand this service.  The partners we already work with at the Community Hub, such as Guideposts and Home Instead, have provided expert advice and support for our clients which they may not otherwise be able to access.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Funding will become an issue as the Memory Cafe expands, particularly to cover venue rental. Other than that I think the main issue is the time available to expanding our services and then training potential volunteers.

2. Actions

  • Support the growth of the Memory Cafe

To continue to advertise the group and to be actively involved in the organisation and delivery of the monthly cafe.

Carry out activities as set out in the action plan, to make the Town Hall a dementia friendly place. The services available through the Hub will therefore be more accessible.
