1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Rochford District Council's role is to support the Rochford Parish Dementia Action Alliance (in process of setting up) and to deliver its Action Plan to help improve the lives of people living with dementia and their carers. Through working with the local Health & Wellbeing Board and local groups supporting residents living with dementia we will ensure our work remains relevant.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review our actions / commitments on a quarterly basis and report back to the DAA.
