1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

I am keen to improve my knowledge of dementai and dementia awareness which I beleive will enable me to provide enhanced care for my patients. Many patients ask me about dementai and I want to be able to give them up to date advice and if required direct them to the appropriate agencies and care providers.

I would like to attend the local 1 hour Dementia Friends Awareness session which would help me to become more up to date with current information about dementai awareness

I am willing to give a talk at one of the Halton DAA events to raise awareness of chiropractic issues related to fragility/older people/people living with dementia. This would included improving musculo skeletal health of carers of sufferers of dementia.

Undertake an environmental audit of your clinic premises to enable the environment to be ‘dementia friendly’ for any patients living with dementia.

I will also talk to my professional association and the Royal College of Chiropractors to raise awarness in my profession.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

I have no other staff at this time but I would be keen for any future staff to be trained in Dementia Awareness .

I will be audting the building and signage to provide a dementia friendly environment. I have signage contraints in the building but as the Alzheimers Society is also located in my building I feel that our shared entrance and protocols is suitable. I will consult with them regarding any issues when I review my work place setting.

I also treat childrem with special needs and I am used to reviweing my practice space according to their needs ,some of which compare to those of dementia patients.
