1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Age Space aims to provide the best single resource and guidance for families to support their elderly parents to enjoy as independent, healthy and secure old age as possible, and receive the best affordable care should they need it.

We want to provide people across the UK and locally with the tools and information to make the very best care decisions.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Understanding of dementia;

Navigating the way through the health and social care systems.

Feeling isolated and unable to make decisions about the best care solutions.

2. Actions

  • Promote and publicise Norwich City DAA partners and actions

    We will act as a media partner for the Norwich City DAA and through www.agespace.org we will promote the activities of the Norwich City DAA and its partners to raise awareness and to encourage more businesses and individuals to get involved.

  • Create a Dementia area on www.agespace.org for all local resources

    We will create a dedicated section of www.agespace.org to Dementia to detail local services and support, and signposting to the best advice and information.

  • Become a Dementia Friendly organisation

    Through training, to become a Dementia friendly champion and organisation so that we can better promote the work of the DAA in Norwich and Norfolk.
