1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

We will ask our patients how they feel our surgery could be made more dementia friendly.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review the effectiveness of our actions by discussing feedback at team meetings.

We will arrange Dementia Friends sessions on different days of the week so that all can attend.

  • Awareness

    Arrange Dementia Friends sessions for all existing staff and new staff

  • Point of contact

    One member of staff (Laura) to be main contact for all staff.  They will attend Bristol Dementia Action Alliance meetings.

  • Gather feedback from patients/families living with dementia

    We will ask our patients and families living with dementia to review our Dementia packs and information and act on the feedback we receive.

  • Dementia Friends Sessions

    We are committed to having Dementia Friend Sessions for all our staff.

    Tony has just delivered a session on Friday 18th January to 10 members of our team, the one before was held on Wednesday 30th May 2018.

    Our aim is to continue offering these sessions to raise awareness for all our staff 2/3 times a year on different days of the week ensuring all the team are able to attend.

  • Promoting Dementia Awareness

    We have added  to our new practice website that we are a Dementia friendly Practice and are in the process of adding a link to your website.

    We have asked BDAA for some posters for us to display on our reception notice board/tv.

  • Ongoing Actions

    Currently gathering feedback to send to BDAA.

    We continue to discuss what we have learnt at team meetings.

    We will revisit our list of Dementia Patients and send out questionnaires to those that have been added in the last 2 years.
