1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Home Instead is committed to changing the face of ageing. With the rise in numbers of people living with Dementia it is important for the whole community to be aware of the issues commonly experienced when carrying out daily activities in the community. Our caregivers will communicate with any clients who are living with dementia and their families about how they are affected and will discuss common issues with other DAA members to try to find ways of overcoming challenges and improving lives.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

We will review the effectiveness of our actions through feedback from our clients, other DAA members, local businesses and organisations and the community.
