Kettering General Hospital

Kettering General Hospital is a medium-sized district general hospital which provides a wide range of services to about 300,000 people in North Northamptonshire. The hospital has served its local community since 1897. It became an NHS Trust in 1994 and a Foundation Trust in November 2008.

1. Action Plan

1. The National Dementia Declaration lists a number of outcomes that we are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Kettering General Hospital has a Dementia Steering Group which has developed an action plan of what they hope to achieve to improve care for patients with dementia. The Dementia Steering Group objectives are to provide care for patients with dementia in line with national guidance and patients and carers report a positive experience of their care at KGH. The Dementia Steering Group provides an important role in assuring that effective structures and systems are in place to implement the requirements of the national and local agenda regarding dementia care across the trust.

The Dementia Steering Group aims to achieve outcomes from the following actions:

  • The Trust will design dementia care pathways, including best practice on discharge:
  • The Trust will improve the quality of patient care and experience of patients with dementia with implementation through an agreed approach to education and competency.
  • Determine an education and training programme that ensures staff caring for the person with dementia understands specific care needs.
  • Implement the ‘butterfly’ system for identifying and supporting the patient with dementia
  • Implement a process of ‘signposting’ carers of patients with a diagnosis of dementia to support and guidance
  • Review environments in which patients with dementia are nursed to inform capital improvement programme

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Challenges to delivering the outcomes include:

  • Ensuring CQUIN dementia assessment is completed for over 90% of over 75 year olds
  • Ensuring that patient profiles are completed as part of the ‘Butterfly scheme’
  • Ensuring dementia is a key local priority
  • Encouraging all staff and others to embed dementia care work for the benefit of patients via a timed work programme taking into account National Dementia Strategy, national dementia audit and NICE guidance

2. Actions

  • Activities for patients

    Orientation boards, activity boxes and memory boxes supplied for 11 Kettering General Hospital Wards during 2012. Continued roll out to other wards during 2013/2014. Orientation boards audited every 6-12 months.

    Second Quater Update 2013 Audits completed on orientation boards June 2013 .Audit in planning stage for use of memory boxes and activity boxes for end of september.

  • Continue dementia care training sessions

    Dementia Care Training sessions for staff have been on going for the last 5 years at Kettering General Hospital by an Alzheimer's Society approved trainer. Further training sessions available through staff development, University of Northampton and internal trainers 2013/2014 programme.

    Second Quarter Update 2013 traning continues for dementia awareness by approved trainers and KGH intranet site includes dementia training packs/information available for all staff to download. new doctors have been trained in the use of butterfly care and the patient passport.August 2013

  • Red tray and red jug system in place for patients who require extra support with feeding and drinking

    Second Quarter Update 2013 Red Tray Scheme for patients that require assistance with meals/eating is used throughout the trust.Red lids on water jugs is being rolled for patients that need support with fluid intake and there are plans to use red beakers for cold drinks for those patients that need support with fluid intake.

  • Introducing suitable feeding equipment

    Adaptive feeding equipment available since 2012 from medical equipment library to assist patients who cannot manage standard plates, cutlery, cups. Audited May 2013.

    Second Quarter Update 2013 adaptive feeding equipment is being used throughout the Trust for patients that require adaptive cutlery, dycem place mat or plate guard to enable them to feed themselves.Staff book out the pack form the medical equipment library for the individual patients following an assessment of their needs.

  • Working with external organisations

    Working closely with local external organisations such as Alzheimer's Society and patient carer representatives to share best practice and improve care in hospital and at discharge.

  • Devise appropriate methods of capturing patient and carer experience
