1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Lee on the Solent Medical Practice is committed to working towards supporting Lee on the Solent to become a successful dementia friendly community. 

As part of ongoing care, we will endeavour to provide the information and access to services to best suit individual needs.  As a GP Practice we will look to support an internal awareness within our clinical support teams. 

All staff at Lee on the Solent Medical Practice will complete dementia friendly training to support our patients and their carers and families to provide a supportive environment.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

Awareness :- Time will always be a challenge to ensure that all the team has dementia awareness. 

Integration:- Communication and integration between all care provider organisations in delivering a complete medical record. Thus it would be good to have all IT systems and links the same but this is something that is being worked towards.
