1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

Our business is Health and Wellbeing which is important for all and customers with Dementia come under the 'all' category.

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

 How would you want to review the effectiveness of these actions?

 Getting the funding for the outreach programme is the first major hurdle.

  • Staff training

    Our local Dementia Action Alliance co-ordinator is to attend our January staff training session and give staff a talk on Dementia awareness

  • Exercise referral scheme

    We are applying for funding to make our exercise referral scheme an outreach project to take the scheme to the community.

  • Supporting people with dementia and and their carers to attend the excercise referral scheme

    The instructor will contact the customers if they have not attended their activity for a while to remind them of their exercise regime
