1. Action Plan
1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?
Admiral Nurses are specialist dementia nurses who give expert practical, clinical and emotional support to families living with dementia to help them cope. They are registered nurses, and have significant experience of working with people with dementia. We link in with other health and social care services, not only in the B&NES area, but also nationally.
The Admiral Service that I work with has been put in place by the Royal British Legion, so our service is targetted at veterans and their families in the Bath, North Somerset and Somerset areas.
2. What are the challenges to delivering these outcomes from the perspective of your organisation?
Veterans and eligible families, as well as other health and social care services, not being aware that this service exists.
Other services/people who may refer cases to us, not being fully aware of the Admiral Nurse role and the difference between us and other services.

To ensure that carers of PWD are supported
The Admiral Nurse service provides support to carers of PWD (including secondary carers i.e. friends and family), ensuring that their needs are met as well as the carers. We are able to offer practical support including, making referrals to other services, practical advise in manageing the different ways in a PWD may present, acting as an advocate and safeguarding etc. We are also able to offer emotional support, regularly making contact with carers and spending time talking to them in a safe space.- Status:

Linking in with other Dementia Services in the area
Making other services in the area (not necessarily those aimed at PWD) aware of the Admiral Nursing Service within the Royal British Legion. We accept referrals from other professionals as well as friends, carers and self-referrals.
We frequently signpost to other services and would like to be a provider that other services think of. There are also other aspects of support that the RBL can provide outside of the Admiral Nursing Service, which we want to continue to make veterans and their families aware of.

To provide support and education to other professionals
We are currently making contact with GP surgeries, mental health teams as well as other services to advise people of our service as well as provide educational support around dementia and the issues both cares and PWD dementia face.
