1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

How will you establish what actions are important locally for people affected by dementia?

We will promote joint working with all interested parties

We will support and promote the work of the Dementia friendly Communities

2. What are the challenges to delivering these outcomes from the perspective of your organisation?

How would you want to review the effectiveness of these actions?

By holding a dementia steering group within the Hospital
