1. Action Plan

1. The National Dementia Declaration lists seven outcomes that the DAA are seeking to achieve for people with dementia and their carers. How would you describe your organisation’s role in delivering better outcomes for people with dementia and their carers?

How will you establish what actions are important locally for people affected by dementia?

To improve the relationship between the services that provide care for patients with dementia so we can learn from each other.Ask our patients what support or services they would find beneficial.




2. What are the challenges to delivering these outcomes from the perspective of your organisation?

 How would you want to review the effectiveness of these actions?

2. Actions

  • Improve our care for our dementia patients

    What we are going to do: we are currently following the iSPACE programme to improve the care for patients with dementia.

  • Improve dementia diagnosis

    What we are going to do: education for practice staff to aid in improving the rate of diagnosis. Improving the information available to patients at the practice.
